Program Director-Care On Call - The Program Director of the Care On Call is responsible for the full administration of this entrepreneurial program of Catholic Charities, Diocese of Joliet.  The Program Director will market and sell the program services to the potential clients, ensuring that all client service targets required for the expansion of the program are met.  In addition to sales and marketing of the program, general duties of the Program Director include screening and hiring of Care On Call Associates, supervision of all direct program staff, oversight of the program budget – income and expenses, quality assurance activities regarding program services, and Care On Call policy development and implementation. Requires Bachelor’s degree in Business, Social Work, Health, Human Services, or other related degree, 3 years experience providing services to older persons, 5 years supervisory experience, & valid IL Driver’s license. Housing Project Manager – The project manager is responsible for the successful coordination and implementation of the administrative and support requirements for housing needs associated with Transitional and Permanent Supportive Housing programs at Catholic Charities.  The Project Manager is responsible for securing housing, landlord relations, intra-program and interagency communications with clients Case Managers or Case Manager equivalent, and other similar entities involved in coordination of  the services.  Qualifications include HS diploma (Associates degree preferred), at least 3 years experience in an administrative or program position in a social services setting, & valid IL drivers license.  Position is Full-Time and located in Villa Park, IL/

Care On Call Regional Manager - Conduct comprehensive needs assessments and provides coordination of Care on Call services to assist older persons to remain in their own homes as long as is medically safe to do so. The Regional Manager meets with family caregivers and physicians to discuss needs and concerns of the client. Assists clients in problem solving and acts as an agency ambassador in the communities served.  Requires RN or BSN or BA/BS in Social Service, 5 year experience in Aging Services, 3-5 years supervisory experience in aging services or related medical field, plus valid IL Drivers License. Work location is Kankakee area, Part-Time, Monday, Wednesday, & Friday 10am-3pm

Case Manager I or II - Function as lead case manager, conduct comprehensive needs assessments and provide coordination of services to assist older persons to gain access to and receive needed formal and informal services. Act as an agency ambassador in the communities served, providing community education, program and agency presentations to interested groups. Acts as support for case management staff in the absence of the Senior Services Assistant Director. Req. RN or BSN, or BA/BS Degree in Social Science, Social Work or related field. 1-5 years experience, valid IL Drivers License.  Work location is Kankakee area, Full-Time, Mon-Friday.
